Employment Type: Permanent, Part-time (0.3 FTE)

Join VON Canada as a Registered Practical Nurse providing client and family-centered, evidence-based nursing care in home and community settings. You will support clients including medically fragile children, adults with chronic/complex care needs, palliative care, and more.

Key Responsibilities:

•  Deliver predictable care according to established nursing plans; consult RN for complex cases.
•  Manage, facilitate, and evaluate care via nursing care plans.
•  Provide thorough assessments of clients and families, document accurately, and report outcomes.
•  Implement strategies from care plans and adjust visits as needed.
• Educate clients and families to promote self-care and independence.
•  Collaborate with multidisciplinary teams, contribute to care plans, and mentor colleagues.
•  Follow quality, risk management, and OHS frameworks.

Requirements:

•  Licensed Registered or Licensed Practical Nurse in the province of practice.
•  Experience in community health and clinical practice is an asset.
•  Strong assessment, communication, and interpersonal skills.
• Ability to problem solve, work independently, and manage time effectively.
•  Personal vehicle with valid license and insurance.
•  Ability to wear PPE and work flexible hours.
•  Current Criminal Records Check; Vulnerable Sector/Child Abuse Registry Check may be required.

Work Schedule:

• 22.5 hours bi-weekly, daytime shifts.
